Our School

Presently, our school nurse, Nurse
Jackie, is assigned to Sandburg every Thursday.
Our Health Assistant, Debbie, is on site every Monday, Tuesday,
Wednesday and Friday. Emergency first aid is
rendered when necessary. However, the school does not have
facilities to care for sick children, particularly those in the
beginning stages of a communicable disease. Parents should keep
ill children at home. Children who become ill suddenly at school
or who are injured at school will receive every care, service,
and consideration. Parents will be notified immediately. It is
the parents’ responsibility to have accurate phone numbers on
file at all times in the school’s office and to respond to
school calls. To call the

Immunization |

NO SHOTS NO SCHOOL - It's California Law
The
California School Immunization Law requires that children be
up-to-date on their immunizations (shots) to attend school. The
following is the number of immunizations required to enroll.

Do not confuse this one
with Hepatitis B, these are different vaccines for different
purposes. This is not required for Kindergarten entry.

There should be one date
and stamp or verification of the disease |

|
|
|
|
Child Health and Disability Prevention (CHDP) |
|
The
California Public Health Department
requires each student to
have a physical prior to entry to first grade. You may pick up a
copy of the green
CHDP form from the school nurse's office or by
downloading the form
here.
This is different than the shot record that was required for
Kindergarten entry. This is a full developmental physical
performed by a physician or nurse practitioner within eighteen
months of your student's entry into first grade. This is done to
ensure that all California children receive early diagnosis and
treatment of disabling conditions. |
